  • Why was Big Tree Medical founded?

    Big Tree was founded by a team that believes there are plenty of providers and plenty of patients who need care, but the current healthcare model makes it difficult for patients to get great care. Through a membership model, a team approach to care, and mobile technology, care providers and patients can have aligned incentives and get a healthcare experience that is unmatched.

  • Who is an ideal Big Tree candidate?

    A nurse practitioner who is looking for a way to care for more patients in a cutting-edge environment. We prefer providers with prior primary care experience. Working with Big Tree makes a great side hustle or a great full-time career.


    Some cases require an emergency room visit or we will send some patients to urgent care when a physical exam is critical. Too often patients don’t know what to do, and they don’t have anyone looking out for their interests. Now you do.


    We are a primary care clinic, and like any primary care clinic, we can handle every situation. Heart attacks, difficulty breathing, strokes, and similar situations require care that you can only receive at a hospital. Surgery, colonoscopies, and other physical procedures are pretty hard to do over a text message. Most of our patients use us as part of a healthcare package that includes something like insurance to cover catastrophic health needs.   • What if I need a collaborative physician?

    Some states require you to work with a collaborative physician. You are free to find your own, hire your own, or work with ours! For $5 per patient per month, you can use our partner physician, who is licensed in most states. Collaboratingdocs.com is a great resource to find physicians in your area.

  • Do I need clinical experience?

    Probably. We have had a few thousand applicants and we find that clinicians with at least a year of experience do better. If, however, you see that your state does not yet have a provider, we would still encourage you to apply if you have a collaborating physician who is willing to work with you.

  • How do I acquire patients for my panel?

    Big Tree will embark upon a digital marketing campaign once you begin working with us. We work with organizations that want to utilize direct primary care as a part of their healthcare solution but are limited by the lack of providers. We connect those patients to you. We are also actively working with businesses, insurance administrators, and related organizations to acquire patients throughout the US.

  • How quickly will my patient panel fill up?

    It is difficult to say. This is a new business landscape that is evolving quickly. Our most successful providers have anywhere from 50-150 patients after 6 months. Some providers only have a couple of patients. Like anything else, the more you put in, the more you get out.

  • How busy will I be?

    We find that for every 100 patients on your panel, you will have about 1-2 virtual visits and 2-4 text message conversations per day. The smaller your practice, the more variation you can expect. These numbers also vary based on practice style.

  • Can I work three days a week?

    No. You are looking for urgent care, where you are paid for a shift. We pay you to take responsibility for patients, not shifts.

  • If I already have an independent practice, can I contract with Big Tree for a panel of patients as well?

    Yes. If you take health insurance in your regular practice, you may want to create a new business to work with Big Tree, which is something you may want to talk to a lawyer about.

  • Can I open a physical clinic?

    Yes. This will make you eligible to take patients that enroll in Big Tree Unlimited, our in-person plan.

  • Do I have to be an F.N.P.?

    Yes, or a P.A. that is comfortable with children. Kids are a part of the program, so if you can't take care of kids, you should not apply.

  • What if I am a doctor, is there a way for me to join?

    Yes! We would recommend working with our partner organization, collaboratingdocs.com.

  • What about medications and labs?

    We are the healthcare hackers. In our model, you get to help your patients figure out how to spend less and still receive great care. We offer affordable access to labs, generic medications, specialty medications, and more. 

  • Is there a medical acuity triage, or do I take every BTM member in my practice area?

    It is your business, so like any healthcare provider, you are allowed to turn people away, though few do so. You are expected to be the primary care provider, not the sole healthcare solution for your patients. Every primary care provider refers patients to specialists while continuing to manage their more basic healthcare needs. The more complex a patient is, the more of a team-based approach you will take to management. This is no different than any other primary care practice.

  • What if I cannot take care of the patient virtually?

    No primary care clinic takes care of everything. Most primary care clinics refer patients to urgent care every day simply because they don’t have time to see them. We send far fewer patients to urgent care than they do simply because we can see all of our patients that need care. You will refer patients to specialists. You will refer to the ER and even urgent care. Every primary care clinic does. But at least your patients will know they need to get their abdominal pain examined at urgent care. Without you, they would just have to guess. 

  • What if I have a really difficult or very sick patient?

    It is no different than if you had a very sick or very difficult patient in your office. You may need to call 911. You may need to refer them to a specialist or direct them to the ER. 

  • Does Big Tree provide benefits?

    Because you are an independent contractor, we cannot put you in a group medical plan, or group 401k. But, Big Tree is on the front lines of fixing healthcare, so we have a solution for you. We can connect you with the low-cost solutions that we connect self-employed people with every day. The cost of health insurance should not inhibit anyone from chasing their dream job. Now it doesn’t have to.

  • Do I have to work 24/7, 365 days a year?

    Not necessarily. You are responsible for your patients 24/7, 365 days a year. Just like any clinic, there will be natural ebbs and flows of care volume. You can team up with another nurse practitioner and cover each other when one of you is unavailable. You can also hire a nurse to take the first layer of calls. 

  • Can I practice how I want to?

    It is your practice. Through Big Tree’s marketing, you will be featured as someone that is practicing mainstream, evidence-based medicine. If you love being up-to-date on the most scientifically sound ways to practice medicine, you will fit right in. We typically lean on the protocols in resources like UpToDate heavily.

  • What does documentation look like?

    We do not bill insurance companies based on your documentation, so the documentation merely serves the purposes of record-keeping. Your clinical judgment determines what is in your notes, but most providers spend a few seconds on each note. There is no billing or coding.

  • Do I need to be tech-savvy?

    No. If you can work a smartphone, email, Facebook, Google, and similar programs, you will be fine. You will need a working laptop, smartphone, and internet connection that supports video conferencing. Most providers use an iPhone, a laptop of some kind, and their regular home WiFi.

  • What billing and administrative tasks am I responsible for?

    Big Tree Corporate will take care of billing and collecting, and we assume those associated risks and costs. We pay you for the patients on your panel. There are admin roles that are similar in scope to any medical practice, but they take about 10% of the time that is spent in a regular practice. You will have some administrative tasks associated with running your own business and we recommend the use of a good small business accountant or bookkeeper. Providers work with the Big Tree billing staff to make sure membership info is up to date.

  • Who covers my malpractice insurance, and is specific telehealth malpractice insurance necessary?

    Nearly all malpractice carriers now have a separate form to fill out for telemedicine. Your existing malpractice will likely be sufficient. Whether you have malpractice through a health system you work for or do not have any coverage, we can help you find an appropriate policy or let you participate in our group policy. Malpractice coverage cost varies by state but is usually under $5,000 per year.
